Social Security Organisation (Socso) also known as Pertubuhan Keselamatan Sosial (Perkeso).

The company offers socio-economic security to all working Malaysian citizens. All the company business in Malaysia has to register to Perkeso as a member and get the member registration number for the monthly contribution. Documentation to be really for membership application including:

1. Form 1 - Registration of employers form (to be submitted in 3 copies with original company stamp and signature)
2. Form 2 - Registration of Employees Attached with employees Identity Card copies.
4. A copy of Director Identity Card. (Either one Director)
3. Photostat Copies of Certificates (Enclose two copies as follows)
i. Limited Company
Form 9 (Company Act 1965)
Form 49 (Company Act 1965)

or
ii. Sole Proprietor/Partnership
Form D (Registration Declaration)
Form A (Business Information)

or
iii. Lawyer Office
Photostat Copy of Letter /Document from Council

or
iv. Clinic
Form 12 (Medical Act 1971-certification from MMA)

or
v. Audit Office
Company Act 1965 - Auditor's Approval

It's better to attend to the office for membership registration. You can get the membership number on the spot from Socso counter. Other wise, you can mail or courier your application documentary and the membership number, certificate, contribution schedule and new Form 8A will send to you within 2 weeks.
